Requesting Your Health Records

You have the right to request access to your health care records.

To request your records, please submit a written request by email to:

Please include:

  • Your full name and date of birth

  • Dates of service

  • The specific records you are requesting

Requests will be processed in accordance with applicable state and federal laws.
